CORPORATE CARE’S BOCES EMPLOYEE ASSISTANCE PROGRAM PACKAGE INCLUDES,
BUT IS NOT LIMITED TO, THE FOLLOWING:
1. EVALUATION OF PROBLEM, SHORT TERM COUNSELING AND REFERRAL (IF INDICATED)
Evaluations of presenting issues are performed by qualified, master’s level counselors. When short-term counseling and/or conflict resolution is indicated, the counselor will work towards that goal. There is no set limit on the number of counseling sessions available. If a referral is indicated, it will be based on the individual’s needs and their approval. Approximately 15% to 20% of all individuals seen are referred for outside treatment.
2. FAMILY MEMBERS INCLUDED
Family members include the employee, spouse and dependent children. Given that the term "family" takes on many meanings today, we are flexible when determining a "family member" (significant others and their children, parents, etc.) We also cover terminated and retired employees.
3. 24 HOUR COVERAGE
If an individual needs to speak with a counselor after normal working hours or over the weekend, they just need to call the office number and they will be given the home and cell numbers for Peter Pontius, President of Corporate Care. Individuals in crisis should never hesitate to contact Mr. Pontius. That is made clear during the "EAP Orientation" sessions.
4. EMPLOYEE ORIENTATION SESSIONS
Corporate Care will present short (10 minutes) information sessions to all employees. These sessions can be presented as many times as is necessary to reach all employees. This has proven to be a very effective and time efficient method of informing employees of the benefits and services provided and how to access them.
5. SUPERVISORY TRAINING
Corporate Care will provide trainings for all supervisors and managers to instruct them as to how the EAP can be used to help them deal effectively with under productive employees. This training lasts approximately one hour. Topics include how to: recognize job performance problems, document the problem behaviors, constructively confront the employee and how to access the EAP if necessary. These trainings will be presented as often as needed at the workplace.
Corporate Care insures employee and family member confidentiality. All who seek counseling are guaranteed complete anonymity. Only by signing a formal consent to release information will any information be shared. Even then, only agreed upon information will be released to the person indicated on the release. The client/professional relationship is protected by laws of confidentiality.
7. REPORTS ON SERVICES DELIVERED
Corporate Care will provide reports concerning the following on a annual basis (unless instructed otherwise):
A. Number of employees and family members seen
B. Total number of counseling sessions throughout the year
C. Types of problems seen
D. Number of referrals made
8. FINANCIAL AND LEGAL ASSISTANCE
Corporate Care has a professional relationship with an attorney group who is available to answer legal questions employees or family members may have. In order to protect clients from being exploited, the question is given to Peter Pontius (EAP) and he contacts the attorney and gets back to the client. That way no clients are contacted by lawyers they may not want to communicate with. Budgeting matters can be taken care of in regular counciling sessions. For more serious credit problems we have professional financial experts at our disposal to answer questions and make suggestions.
9. WEBSITE SERVICES
The Corporate Care website is always being updated. The services available on the site include but are not limited to the following:
More than 250 informational articles on topics such as personal finances, mental health issues, family and marriage, work/life balance, parenting, safety, retirement funds, personal and family health, etc.

- Power point trainings on topics including Workplace Harassment, Alcohol and Substance Abuse/Awareness, Stress Management, Healthy/Unhealthy Relationships, Men's Health Issues, EAP Supervisory training, EAP Orientation, Working With Multiple Generations, and others

10. COUNCILING BY SKYPE AND E-MAIL
If employees/family members can't find time to get to one of our 7 office locations, or if they are more comfortable using social media, they can make arrangements for appointments via Skype. Counseling is also available via E-mail and Instant Messaging.
11. OFFICE LOCATIONS AND OFFICE HOURS
Presently Corporate Care has 7 office locations: Geneva, Clifton Springs, Canandaigua, Newark, Penn Yan, Waterloo and Auburn. Counseling sessions are available mornings, afternoons and evenings.
12. HOUSE CALLS AND WORKPLACE CALLS
If for some reason an employee/family member can not get to one of our offices, thay can be seen in their homes or at their workplace. The circumstances are usually health related for the home visits. Meeting at the workplace happens for any number of reasons.
13. PROMOTION OF SERVICES PROVIDED
Corporate Care will work towards employee knowledge of available services via: business sized information cards for all employees, employee orientations, fliers, and posters for all locations. Other methods will be utilized at the request of the employer.
14. WELLNESS TRAININGS
Corporate Care will provide free wellness trainings on topics such as: Stress Management, Nutrition, Low Fat Cooking, Men's and Women’s Health Issues, Drug/Alcohol Awareness, Parenting, Healthy/Unhealthy Relationships, Dealing With Difficult People, Cultivating a Civilized Workplace, Effective Communication, Working With Various Generations, and many others. These can be done at the workplace.
15. WORKPLACE HARASSMENT AND WORKPLACE VIOLENCE TRAININGS
Given the increase of both workplace harassment and workplace violence, many companies are taking advantage of these trainings. They inform participants how to recognize these issues and how to effectively prevent or deal with them. Each training takes approximately one and a half hours. These can be done as often and for as many shifts as requested by the employer.
16. ON-SITE MANAGEMENT CONSULTATION SERVICES AND EMPLOYEE MEDIATION SERVICES
Corporate Care will provide consultation to managers/supervisors on any issue related to human relations problems. This often includes providing mediation services between employees not getting along or meeting with entire departments to analyze what is going wrong and then make recommendations as to how best deal with it. Also assist with customer service issues and provide indicated customer service skills training.
17. CRITICAL INCIDENT DEBRIEFING
In the event of a critical incident, Corporate Care will provide debriefing services/sessions for those involved. All human resources personnel have the home and cell numbers for Peter Pontius (EAP). If there is an emergency after hours, just call the numbers and we will be sure there is someone there to help people deal with whatever is happening when you need them there.
18. PERSONAL LIFE ASSISTANCE
Corporate Care is available to help employees/family members with any kind of assistance with personal life issues (best way to make airline reservations, suggestions on what to buy a spouse, best restaurants in the area, reviews of customer products, dating questions, etc.).
19. PREPARING FOR THE COLLEGE EXPERIENCE (INDIVIDUAL OR GROUP)
Preparing to send a child to college is a very stressful and potentially confusing time. We can help people sort out the financial aid and admissions process as well as understanding all aspects of the process. This includes helping parents prepare for the move in, separation anxiety, homesickness, etc.
20. HYPNOSIS FOR RELAXATION, STRESS MANAGEMENT AND ASSISTANCE IN QUITTING BAD HABITS
Peter Pontius has been using and teraching hypnosis for more than 20 years. it is a valuable tool to help people make changes in their lives. It is especially helpful when used in conjunction with other traditional therapy, but can be used either way.
21. ASSISTANCE WITH CHILD AND ELDER CARE ISSUES
Corporate Care will assist employees who are seeking child care services as well as elderly and/or ill parents. We also offer to facilitate on-site "Sandwich Generation" support groups and trainings.
22. PERSONAL AND PROFESSIONAL LIFE COACHING
Sometimes people don't really need counceling, but rather someone to help them navigate through life. This includes helping with decision making skills, parenting, getting through divorce or loss, etc. In the workplace, managers are sometimes in need of guidance with how to deal with new responsibilities, functioning within the structure of the department or company, improving interpersonal skills, or anything else that may help them be better managers.
23. "BALANCING WORK AND LIFE" PROGRAM
This includes self help and professionally directed activities and information such as: Personal Health Assessments with Recommendations for Follow-Up, Avoiding Debt, Getting out of Debt, Avoiding Financial Scams, Budgeting Worksheets and Professional Follow-Up, Paying for College and Understanding the Truth About Financial Aid, Preparing for the College Experience (Workshops and Individual Assistance), Saving for Retirement, Planning for Retirement, Assistance in Finding Primary Care Physicians and Specialists, Caring for Elderly Loved Ones, Caring for the Caregiver, Understanding Illnesses of the Elderly, Dealing with Uncooperative School Districts (will even accompany parents to meeting with school officials), and other topics. New topics will be added as needed.
24. TOPICAL ARTICLES FOR POSTING OR INTERNAL E-MAIL
Throughout the year articles of interest will be sent to your Human Resources Department. They are formatted so that they can be copied and posted on bulletin boards or shared through an internal e-mail system to all employees. Recent articles have focused on new age drugs to be aware of, adolescent trends that are dangerous, depression, working with difficult people, and other topical issues.
25. TRAININGS AVAILABLE ON-LINE
Topics such as Harassment, Violence in the Workplace, EAP Orientation, EAP Supervisory Training, Caring for Elderly Loved Ones, Stress Management, Cultivating a Civilized Workplace, Working with Different Generations and other pertinent topics. These will be available by Fall 2015.
IF A COMPANY CHOOSES TO INCLUDE THE FORMAL REFERRAL PROCESS, THE FOLLOWING SERVICES ARE INCLUDED
(FREE OF CHARGE).
1. SUPERVISORY TRAINING
All supervisory personnel will be offered training on the effective use of the EAP when dealing with an under productive employee. This training includes the following topics:
A. How to recognize job performance problems
B. How to document unacceptable job performance
C. How to “Constructively Confront” the employee
D. How to make a “Formal Referral” to the EAP based on job performance.
2. ON-SITE CONSULTATION FOR MANAGEMENT PERSONNEL
An EAP professional will meet with any manager, upon request, to deal with individual or group issues at the workplace. We will work towards collaboration between management and the employee to establish a plan for problem resolution.
3. FORMAL REFERRALS - INFORMATION WHICH IS SHARED WITH THE EMPLOYER
Corporate Care will provide the company EAP representative with the following information (provided a release of information has been signed by the employee):
A. That the evaluation time has been scheduled
B. That the employee did or did not keep the appointment
C. Whether a referral for additional services is necessary
D. Whether or not the employee continues with recommended treatment
E. That the employee has completed or failed to complete recommended treatment
